CASEWORKER-FULL TIME

PWP Health provides community-based services to consumers living with serious mental illness. We are currently in search of high performing Caseworkers to work at our Chicago IL location!

The ideal candidate must have excellent communication and documentation skills and possess an understanding of all mental health. The Caseworker will play an active part in the interdisciplinary team.

Caseworker responsibilities include: 

  • Participating in the person-centered planning process. Providing direct home-based behavioral health services and supports, including therapy for the family.

  • Providing direct services for the family as indicated in the Person-Centered Plan.

  • Assisting the team leader in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward goals.

  • Assisting the team leader with consulting identified non-medical providers, engaging community and natural supports, and including their input in the person-centered planning process.

  • Meeting minimum billable units as assigned by the Program Director.

As a Caseworker, you will be hired at one of the below levels: 

  • Qualified Mental Health Professional (QMHP) 

  • Mental Health Professional (MHP) 

  • Rehabilitative Services Associate (RSA)

Caseworkers can bill for the following services: 

  • H2015- Community Support

  • T1016- Case Management: Mental Health

  • T1016 HS- Case Management: Client Centered Consultation

  • T1016 TS- Case Management: TLA ( excluding RSA)

  • H2011- Crisis Intervention ( excluding RSA)

**Please pay close attention to the qualifications (presented below) associated with the level to which you are applying (i.e., RSA, MHP, and QMHP). You will be asked to indicate your level during application submission, and you will be assessed at your respective level during the interview. **

Qualified Mental Health Professional or QMHP – You must meet one of the following:

  • A licensed social worker (LSW) possessing at least a master’s degree in social work and licensed under the Clinical Social Work and Social Work Practice Act [225 ILCS 20] with specialized training in mental health services or with at least 2 years experience in mental health services;

  • A licensed professional counselor possessing at least a master’s degree and licensed under the Professional Counselor and Clinical Professional Counselor Licensing Act

[225 ILCS 107] with specialized training in mental health services or with at least two years experience in mental health services;

  • A registered nurse (RN) licensed under the Nurse Practice Act [225 ILCS 65] with at least one year of clinical experience in a mental health setting or who possesses a master’s degree in psychiatric nursing;

  • An occupational therapist (OT) licensed under the Illinois Occupational Therapy Practice

Act [225 ILCS 75] with at least one year of clinical experience in a mental health setting; or An individual possessing at least a master’s degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or family therapy or related field, who has successfully completed a practicum or internship that included a minimum of 1,000 hours of supervised direct service, or who has one year of clinical experience under the supervision of a QMHP.

  • Any individual meeting the minimum credentials for an LPHA under this part is deemed to also meet the credentialing requirements of a QMHP.

Mental Health Professional or MHP – An individual who provides services under the supervision of a qualified mental health professional and who possesses: a bachelor’s degree; a practical nurse license under the Nurse Practice Act [225 ILCS 65]; a certificate of psychiatric rehabilitation from a DHS-approved program plus a high school diploma plus 2 years experience in providing mental health services; a recovery support specialist certified and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc., plus one year experience in providing mental health services; an occupational therapy assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with at least one year of experience in a mental health setting; or a minimum of 5 years supervised experience in mental health or human services. A supervised internship in a mental health setting counts toward the experience in providing mental health services. Any individual meeting the minimum credentials for an LPHA or QMHP under this Part is deemed to also meet the credentialing requirements of an MHP.

Rehabilitative Services Associate or RSA − An RSA must be at least 21 years of age, have demonstrated skills in the field of services to adults or children, have demonstrated the ability to work within the provider's structure and accept supervision, and have demonstrated the ability to work constructively with clients, treatment resources and the community. Any individual meeting the minimum credentials for an MHP, QMHP, or LPHA under this Part is deemed to also meet the credentialing requirements of an RSA.

Training Requirements:

  • Must Complete All Agency initial orientation training and monthly training courses as required to include: HIPAA, Client Rights, Confidentiality, NCI, First Aid, CPR, and Blood Borne Pathogens training.

  • Service Definition Trainings: 3 hours service definition required components, 3 hours Crisis Response, 3 hours of I.T.P Instructional Elements

  • Training to be completed within thirty days of hire: Person Centered Thinking, Introduction to Motivational Interview, Cognitive Behavioral Therapy

PWP Health provides community-based services to consumers living with serious mental illness. We are currently in search of high performing Outpatient Therapists (OPTs) to work at our Chicago location.

The outpatient therapist (OPT) will work with individuals and groups to promote optimum mental health. The OPT may help individuals deal with addictions and substance abuse; family, parenting, and marital problems; suicide; stress management; problems with self-esteem; and issues associated with aging and mental and emotional health etc.

OPT responsibilities include:

  • Maintain confidentiality of records relating to clients’ treatment.

  • Maintain an hourly billable caseload of a minimum of 25 hours per week.

  • Perform IM + CANS (IATP) assessments and behavioral screening as assigned.

  • Provide supervision for MHP and QMHP level Caseworkers.

  • Encourage clients to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships.

  • Guide clients in the development of skills and strategies for dealing with their problems.

  • Prepare and maintain all required treatment records and reports.

  • Counsel clients and patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.

  • Collect information about clients through interviews, observation, and tests.

  • Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations.

  • Develop and implement treatment plans based on clinical experience and knowledge.

  • Collaborate with other staff members to perform clinical assessments and develop treatment plans.

  • Evaluate clients’ physical or mental condition based on review of client information.

  • Meet with families, probation officers, police, and other interested parties to exchange necessary information during the treatment process.

  • Refer patients, clients, or family members to community resources or to specialists as necessary.

  • Counsel family members to assist them in understanding, dealing with, and supporting clients or patients.

  • Evaluate the effectiveness of counseling programs and clients‘ progress in resolving identified problems and moving towards defined objectives.

  • Plan, organize and lead structured programs of counseling, work, study, recreation, and social activities for clients.

  • Modify treatment activities and approaches as needed to comply with changes in clients’ status.

  • Learn about new developments in their field by reading professional literature, attending courses and seminars, and establishing and maintaining contact with other social service agencies.

  • Discuss with individual patients their plans for life after leaving therapy.

  • Gather information about community mental health needs and resources that could be used in conjunction with therapy.

  • Monitor clients’ use of medications.

  • Plan and conduct programs to prevent substance abuse or improve community health and counseling services.

  • Run workshops and courses about mental health issues.

As an OPT, you will be hired at either a non-licensed or licensed level:

  • Non-licensed: Qualified Mental Health Professional (QMHP)

  • Licensed: LCSW, LSW, LPC, LCPC, Ph. D.

To qualify at the level of an OPT, you must meet the following requirements:

  • Master’s Degree in a human service field and must possess full or provisional licensure status (LCSW, LCPC, LSW, LPC or Ph. D.) OR master’s degree in a human services field (non-licensed) with experience working in community mental health.

  • Behavioral Screening Certification, if required.

Training Requirements:

  • Must complete PWP Health initial orientation training and state mandated trainings.

  • Must complete between 15-20 hours of crisis management training - within the first 90 days of employment.

  • Must complete IM-CANS training modules, practice examinations and final examinations within the first 14 days of employment.

  • Must possess a valid driver’s license.

Additional Qualifier: Must be willing to work primarily on the north and/or west side of Chicago.

PWP Health provides community-based services to consumers living with serious mental illness.

The ideal candidate must have excellent communication and documentation skills and possess an understanding of all mental health. The BHS will play an active part in the interdisciplinary team. This position requires 40 hours per week and 25 weekly billable hours.

Behavioral Health Supervisor responsibilities include:

  • Serves as a back-up to support IATP assessment activities, when necessary.

  • Convening the Child and Family Team for person-centered planning

  • Consulting with identified medical (for example, primary care and psychiatric) and non- medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process.

  • Review/Approve all team member progress notes.

  • Provide weekly supervision to all team members.

  • Create/Review schedules for all team members.

  • Provide routine monitoring or services being rendered by team members for quality assurance.

  • Deliver direct care services.

  • Maintain/Audit all team member consumer charts for 100% compliance.

  • Manage and approve time and attendance.

  • Reports to manager on a weekly basis on progress or regress of team caseload.

  • Ensuring the team maintains a full caseload.

  • Maximizing services for clients needs to ensure continuity of care.

  • Conducting monthly outcome summaries and progress of consumer goals.

  • Maintain 100% authorizations on all consumers within the team.

  • Coordinating behavioral health services and other interventions for the client or other family members with other professionals and Child and Family Team members.

  • Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the IATP.

  • Responsible for ensuring that services are provided in a manner which meets all applicable regulatory licensure and accreditation regulations.

  • Participate in multidisciplinary staffing. Attends all appropriate in-services, workshops, or continuing education programs.

  • Responsible for keeping client medical record charts and their own personnel file information up to date in a timely manner.

  • Participates on interview and orientation team

  • Responsible for admissions and discharge process

  • Work to ensure mental health outreach, education, and prevention in the community.

  • Maintains strict confidentiality according to all Federal and State guidelines and requirements.

  • Exemplifies personal and professional conduct. Upholds the Professional Code of Ethics.

  • Maintains a cooperative relationship with employees, medical staff, and others.

  • Responsible for personal development to ensure current knowledge in the profession.

  • Willingness to work beyond what is required periodically and travel for continuing education hours.

  • Maintain service notation that meets agency notation standards, Medicaid standards, and/or any other auditing agency such as the LME. (For timeliness, quality, and completeness). This notation must include: 1) the purpose of contact; 2) describes the provider’s interventions; 3) effectiveness of the intervention; 4) the time spent performing the intervention; and 5) signature (degree/credentials or position) of the person providing the service.

  • Must be available in the on-call rotation for 24/7- 365 days per year first responder coverage.

  • Must maintain the flexibility to respond to client needs and crisis situations beyond the agency's normal working hours.

  • Deliver services in various environments, such as homes, schools, jails (for state funds only), homeless shelters, etc.

  • Perform any other job-related duties as specified by the supervisor.

Job Summary:

As a Behavioral Health Manager (LPHA) at PWP Health LLC, you will play a crucial role in overseeing and optimizing the behavioral health services provided by the organization. Your responsibilities will include the review of , collaboration with the Utilization Management (UM) team for timely reauthorizations, and day-to-day management of operations to ensure the highest quality of care. In this leadership position, you will directly supervise Behavioral Health Supervisors (BHSs), actively participate in training programs, and contribute to the hiring process for various positions within the behavioral health department.

Key Responsibilities:

IM+CAN Review and Utilization Management:

  • Conduct thorough reviews of IM+CANS (LPHA review) to ensure accurate assessments.

  • Collaborate with UM to facilitate the timely submission of reauthorizations, maintaining compliance with regulatory requirements.

Operations Management:

  • Take charge of day-to-day operations to ensure the smooth functioning of the behavioral health department.

Supervision and Performance Management:

  • Directly supervise BHSs and complete their annual performance reviews.

  • Participate in the hiring process for BHSs, case workers, office staff, and other positions as needed.

Training and Professional Development:

  • Actively participate in training programs alongside the Clinical Program Director, Human Resources, and Compliance Department.

  • Participate in orientation sessions and contribute to the professional development of the team.

Clinical Oversight:

  • Oversee Clinical Treatment Team Meetings.

  • Facilitate team meetings with Behavioral Health Specialists.

  • Conduct monthly clinical supervision for half of the staff, jointly sharing responsibilities with the Behavioral Health Director (BHD).

Administrative Duties:

  • Queue and monitor BHS notes for quality and compliance.

Billing and Financial Performance:

  • Meet billable expectations, with a target of 15 billable hours per week.

Strategic Collaboration:

  • Collaborate with the Marketing Department to manage the incoming volume of referrals, explore new referral sources, and attend events to enhance community visibility.

  • Contribute to the development and supervision of new and innovative clinical services.

In order to apply for this role, you must be a Licensed Practitioner of the Healing Arts (LPHA). To be considered qualified as a Licensed Practitioner of the Healing Arts (LPHA) you can be:

  • A physician who holds a valid license in Illinois and is legally authorized under state law or rule to practice medicine in all its branches, so long as that practice is not in conflict with the Medical Practice Act of l987; OR

  • An advanced practice nurse with psychiatric specialty that holds a valid license in Illinois and is legally authorized under state law or rule to practice as an advanced practice nurse, so long as that practice is not in conflict with the Illinois Nurse Practice Act or the Medical Practice Act of 1987; OR

  • A clinical psychologist who holds a valid license in Illinois and is legally authorized under state law or rule to practice as a clinical psychologist, so long as that practice is not in conflict with the Clinical Psychologist Licensing Act; OR

  • A licensed clinical professional counselor (LCPC) possessing a master's degree who holds a valid license in Illinois and is legally authorized under state law or rule to practice as a licensed clinical professional counselor, so long as that practice is not in conflict with the Professional Counselor and Clinical Professional Counselor Licensing Act [225 ILCS 107]; OR

  • A marriage and family therapist who holds a valid license in Illinois and is legally authorized under state law or rule to practice as a marriage and family therapist, so long as that practice is not in conflict with the Marriage and Family Therapist Licensing Act [225 ILCS 55]; OR

  • A clinical social worker possessing a master's or doctoral degree who holds a valid license in Illinois and is legally authorized under state law or rule to practice as a social worker, so long as that practice is not in conflict with the Clinical Social Work and Social Work Practice Act.

In addition to being an LPHA, we are also looking for someone who has solid experience with:

  • OnTarget or a similar electronic health record (EHR) software program

  • Technology including the use of scanning equipment, Google Workspace (i.e., Google Sheet, Google Docs, Google Meet, etc.)

Knowledge, Skills and Abilities:

  • Read, write, and speak fluent English; excellent verbal and written communication skills (bilingual is a plus)
